Only National and International shipping.

Sadly the hosting where I had my website on just went down for a few weeks with no notice, so I lost everything and Iḿ starting my website again. Before I had it set up so that there shipping options were:

National (Us, Mexico, Canada, Puerto Rico and Hawaii)
If the Total was 59.99 and under it was $3 for shipping and over $60 it was free shipping.

International (Everywhere else)
If the Total was 59.99 and under it was $4 for shipping and over $60 it was free shipping.

I cant figure out how to make it like that again. I would appreciate some help.

Just a quick related question. I'm trying to setup nationwide shipping only for Australia. I found some other threads which suggested removing all entries except Australia from the Countries table in the database. I did this and it works but there's a slight problem. On the estimate shipping page, when you choose a country, there are three entries: Please Choose a Country, a blank line, and then Australia. The Australia items works fine, but if you select the empty line, the States/Provinces dropdown box is populated with states of USA. How do I fix this?

I've checked the countries table and the only entry is Australia.

Any help would be appreciated.

Apologies. I just fixed my problem. In my admin section under Customer details, it still had the default country ID as 223, which is USA. I changed it to Australia and voila! It's fixed :)
