Upgrade Error: 1146 Table 'zc1.TABLE_SAGE_PAY_FORM' doesn't exist

Hi There,

I recently upgraded to d and in the admin section i keep getting this error

"1146 Table 'zc1.TABLE_SAGE_PAY_FORM' doesn't exist
in:
[ SELECT * FROM TABLE_SAGE_PAY_FORM WHERE zen_order_id = '293']
If you were entering information, press the BACK button in your browser and re-check the information you had entered to be sure you left no blank fields."

Could someone please help me out.
