1062 Duplicate entry '1-1' for key 1 in Options Values Manager

Using Version - v1.3.9f
Not an upgrade, fresh installation.
No Addons
Customised images and colours

Trying to add downloadable products. When I try adding the Option Value Manager, I'm getting the error:

"1062 Duplicate entry '1-1' for key 1
in:
[insert into products_options_values (products_options_values_id, language_id, products_options_values_name, products_options_values_sort_order) values ('1', '1', 'Current Image', '0')]
If you were entering information, press the BACK button in your browser and re-check the information you had entered to be sure you left no blank fields."

Any Ideas?
