It appears as indexCategoriesHeading
Might you have a category with that name?
If so, change the cat name as this is in the DB
If it is in a file use the developers tool kit to locate

i search using developers tool kit and it states that it searched all the files and found nothing

In the Developers Tool Kit, do a search on:

It could be you have a header broken and the way it is built is not a direct naming ...
You may get quite a few results, but look through them carefully for something that appears illogical ...

i just did a refresh install of zen 1.5.1 and now i am getting this problem again. i am 100% sure i don't have a category named it.

Look at the file in
includes/languages/english/index.php
or
includes/languages/english/AandD/index.php
Towards the bottom there are 2 defines
define('HEADING_TITLE',
See what follows these

when i change templetes it goes away. also i just went and replaced that index file with the original and it didnt help

well i found the file that is causing it to do it.
Its a overide that i think i put in the wrong section all is good now so far
