create an account emails don't always come

version 1.3.9h I don't always get create an account email when some one creates one. I set admin email create and account to 1 when first setup, have had several people create accounts without notification had several create with notification. Last 2 no email I just created an account an email came. Is there a trick or bug?

Thanks for your help. I have email archive manager installed there is no record of an email being sent to admin for those 2 customers. I show create account emails being sent to admin for other customers. last 6 customers 4 emails sent to admin 2 no emails, but email show they went to every customer, just not to admin.
(When I say admin I mean the email address marked to sent create account notifications.)
