Quiz Form or template

Hi
I'm looking for a simple html code for a quiz form. the form should contain the following fields:

3 Radio Buttons to put text questions

Box with a price characteristics, e.g.

price
colour
size

Address Box with:

Name
Surname
e-Mail
Street
Home no.
Postal Code
City
Country

Submit Button --> output exl.file or just email

That's it and no more. I want to include the code on a EZ-Page.

can anyone help me with this on?

There is also Mach forms and Wufoo, both easy to use and embeddable even in easy pages.
