CUSTOMER UPLOADED FILES LIST AND DOWNLOAD issue

I have added this addon to zen cart and tried using the file upload function. The uploaded files are listed on the Admin-->Customers-->Uploads but when I go to download the file it says
File "DIR_FS_UPLOADS4.jpg" does not exist (251)

what could be the problem of this?
