Add a field in a product listing?

Thanks in advance...

A client needs a form field of some sort in some product listings, for the buyer to give short instructions about embroidery in a product. For example, a cap with a name embroidered on the front, the buyer needs to give the name to be embroidered.

Any ideas on how to get this done?

Thanks, I suspected something like that. I'm having to do a lot of research for this client, his needs are different than my limited Zen Cart experience covers.
