How to set up Standard, Priority, Express Shipping?

I have 3 different shipping methods that I would like to offer each with a set per item price.

For example Standard should be 4.99 for the first item, priority 9.99, and express 14.99 then each additional 2.99 for each.

I have been able to set up Standard easily enough by setting up Per Item shipping, with 2.99 per item and 2.00 handling charge.

It seems like setting up the other 2 should be just as easy, but I can't seem to create more per item shipping methods, it's either 1 or none.

Does this make sense? What am I missing?

for examples on how to do this ...

Make sure the new filename/replaces are like:
item.php to itempriority.php
item to itempriority
ITEM to ITEMPRIORITY

and NOT with underscores such as:
item.php to item_priority.php
item to item_priority
ITEM to ITEM_PRIORITY

Make sure you are not changing CODE or variables such as:

      $item_total_count = $total_count - $_SESSION['cart']->free_shipping_items();

                                                     'cost' => (MODULE_SHIPPING_ITEM_COST * $item_total_count) + MODULE_SHIPPING_ITEM_HANDLING)));
