Help with zj_black template display error

I hope someone can help me out with this issue, my site was running this template (zj_black) perfectly fine until one day it stop working correctly and the main page looks like the attached image,(green bar on the left cant remove it) everything else works good its just the main page displays bad and i can figure out why,

i didnt make any changes or updates

Looks like something you or someone have added to define_main_page.php:

thanks fot the tip i didn't know where to look for this error, aperantly someone change it or hacked into my files, how could i check if my system has the correct security settings

Well, if you didn't put it there, I guess you should assume you've been hacked.
