No phone number field in customer details

Hope someone can help!!
Have recently installed ZC v1.3.9h. I am using the barebones template and installed ceon manual card. All works very well but I cannot collect customer phone numbers. There is no phone number field showing in the 'Customer Details' area or 'Checkout'. However, if I log in as a customer and go to 'My Account' there is a phone number field shown as compulsory in 'change my account information'. This works fine and shows up in the database. I have checked -
includes/templates/templatedefault/templates/tpl_modules_create_account.php includes/templates/YOURTEMPLATE/templates/tpl_modules_create_account.php
They are normal.

Any chance you are using the Classic template? Check in admin under tools-> template selection. The default is Classic Contemporary Green.

If so, check includes/templates/classic/templates/tpl_modules_create_account.php

If so, do you have this code in /includes/templates/classic/templates/tpl_modules_create_account.php around line 118-121?

Thanks for the fast response but may not be applicable. I'm using a template called Barebones not Classic. However, I have checked with the Classic template installed and I do get the same problem. Hmmm I'll have a look at the files.

I have checked my folder/file structure and this file 'tpl_create_account.php' is located in includes/templates/template_default/templates. I have checked 'tpl_create_account.php' and it is complete.
The 'Classic' folder only contains a 'CSS' folder and an 'Images' folder. This is the same for the 'Barebones' template.
I notice that on the Zen Cart Wiki the description of the 'Customer Details' contents does not have a Telephone field. It is identical to my Customer Details which also has no telephone field.
Everything is working perfectly except for this gltch. I really need the phone number as we run classes and it is often necessary to contact students by phone to reschedule classes.
