When adding attributes, it goes to index?

I am having some trouble adding new attributes to a new product in the attributes controller.

When I click insert after selecting the option name and option values, it takes me to the admin index....

I replaced the option_value, option_name, and attributes_controller with the stock files -- still nothing.

Any ideas?
