How to duplicate the "Contact Us" form?

Hey all,

I have what is by now a pretty customized version of Zen Cart and one problem still stands in my way as I get close to being operational...

I am trying to duplicate the contact form found on the contact us page. I am doing a clothing shop that does requests and have created a new main page (not an EZ page), called "Special Requests" where I want users to be able to submit specific requests.

Does anyone know how I can duplicate this form for my requests page? Is it ridiculously difficult? Or somewhat manageable?

I am not an amateur per-say but am still new to Zen Cart and certainly not a pro developer. You can take a look at my page in development at https://www.brokenbird.ca/cart . The requests page is currently blank right now because of an unrelated problem but you should be able to get an idea what I'm going for.

Any help would be greatly appreciated. So far the atmosphere in the forums has been really excellent. I'm excited to join this community.

P.S. I did search the forums / FAQ for an answer but could only find one kind of vague description of what to do. Hence the new topic here.

There are a number of very detailed threads discussing how to create CUSTOM FORMS for zencart, and in truth, they are not easy to do.

The quickest and easiest solution we have found is to create bespoke forms with another program, and to just embed them in an EZ-page.

We use a program called MachForm from Appnitro. It costs a few dollars, but is amazingly simple to use.

We regularly create very complicated forms (in just a minute or two) and embed these into the ezpage using an Iframe. (MachForm will auto generate the iframe code for you).

If you decide to get MachForm, I can almost guarantee that you will have your form embedded into your ezpage within 90 minutes of obtaining the software...

(This form took about 8 minutes to create and about 1 minute to embed into ZC).

Look in the Free Software Add-Ons for "Ask a Question"; that add-on is for the product_info page to allow a customer to ask a question about a specific product.

The basics of the add-on should get you started to where you want to be.
