How to add optional questions at checkout?

Hi,

I have a quick question. I was thinking to include some optional messages with checkboxes (e.g. contact me if an item is sold out) where people can tick at checkout. How do I make sure these options will also get transferred and displayed in the order confirmation receipt/email?
