Easy Populate Help with Column names

Hi i am trying to use easy populate, ive been trying to upload the file but i never get a success message and nothing is added.(i am using excel to edit/save the CSV.) so my two questions are, what is the column field name for Products Model:, and do i need to have the v_products_id filled in?
