New product added but does not appear in Admin or Cart

We have just upgraded to v1.3.9, which we implemented with a clean install. We've tested extensively and the cart is working just fine so far as we can tell.

I have just created a new category and added a new product . I've marked the product In Stock and set a default stock quantity of 999. When I click insert, I am returned to the Categories / Products page for that category, but my product is nowhere to be found and under the ID on the left I see Categories: 0, Products: 0. When I return to Categories / Products - Top, I see an indicator that I have a Linked Product, but I have no way to get to it.

I can see the product in the database table using a MySQL tool. It has no product_id. I can add an ID using the MySQL tool, but it still is not viewable in the admin panel or in my cart. The new category appears in the category table and is viewable/editable from the admin panel.

When I try to add another product with a different product name, I get this error message:

1062 Duplicate entry '0' for key 1

If you were entering information, press the BACK button in your browser and re-check the information you had entered to be sure you left no blank fields.

These products have no attributes assigned (and never have); only price.

I had lots of these issues. Check your PHP version.

Check the following:

Do you have a Product in the tables:
products
products_description
products_to_categories

Does the Product in the table:
products

have a master_categories_id set?

If you add a products_id in the products table, make sure you have it in the products_description with the same products_id ...

Make sure you have a record in the products_to_categories table with the same products_id and a valid categories_id and that categories_id would be the master_categories_id in the products table ...
