How do I set up custom UPS ground rates?

Hi,

I want to set USP like this :

UPS ground, I want to charge a flat rate of $6.95 for all orders under $100, and free UPS ground shipping for all orders over $100.

If customer want 2 day or overnight shipping, then they will pay extra, so those charges need to be filled in.

Is it possible?
