How do I add radio buttons in product page?

Hi i want the product page similar to http://www.wooloot.com/products/Wave-%252d-Blue.html it has radio buttons how to add these in my zen-cart site?

also i have dropdown menu in cart from where buyer will select one option (charity organization) to whom 10% of his/her payment will be donated. e.g i have 4 organization and i want to sort them in admin area in 4 different categories by their name so all the relevant donations are place in the same category.

That would require a lot of custom coding, and needs to be addressed in the thread where you originally asked the question.
