Stuck at ENTRY_SOURCE Do I Reinstall Template?

My create new account page is not showing a <Submit> button and ends at Were You Referred to Us? ENTRY_SOURCE

I see an old thread mentions changing templates and seeing if that is the problem, well it is, I obviously changed something I should not have. My question to everyone is do I reinstall the template or do I have a bigger problem?

Zen Cart does not have a referred To Us in the create account.

Where did you get the template?

I just updated from 1.3.7 to 1.3.9 so the database on new update doesn't have the Reference Mod tables installed, or so I am thinking after your response and my little brain placing all the pieces together. Thanks for the help.
