Addings extra fields to Registration form

Hi guys,

Does anyone know of a addon that will allow extra fields to be added to the registration page?

As we are doing a wholesale site we need extra information thats not available on the standard form. Also a 'upload' form for documents would be great.

Any help will be great

Extra info can be custom programed

It will require DB additions as well to house the additional data
