customer can't create new account

Although I have 20 to 30 new accounts created successfully each day, once or twice a week I have someone who fills in all of the fields, but when they hit submit, they are returned to the home page without creating an account.

Version 1.3.9h

Any suggestions to isolate or solve this problem. Currently, we are asking the customers to clear their browser cache and try again. Sometimes they are successful, sometimes not.
