Reward points PAGE issue.

I have been running the reward points module with great success for the last year. Today I installed the reward points page (I'm running 1.39) but the page does not show up in the define pages editor in the admin and the information box is not displaying correctly. You can see see on the bottom left here (or any page for that matter).

Any advice regarding this would be helpful. THanks!

Reward Points Page is a supplement to Reward Points Full Suite. If you're using Rewards Points Full Suite 2.1a, you will need to upgrade to ZC 1.5 since the Suite has been modified to work on ZC 1.5. However, it's not clear if Reward Points Page will be compatible with the Suite 2.1a. Otherwise, you could try using Suite 1.33a, which is designed for ZC1.3.9.

In that module Reward Points Page there is a file left out. Duplicate the file in includes/languages/english/html_includes and put in Your_Custom_Template folder like this.
includes/languages/english/html_includes/Your_Custom_Template/define_reward_points.php.
That will make the page show up in the define pages editor in the admin.
