delivery date picker- pop up calender problem

Dear Gurus,

I have downloaded code of "Delivery date picker" form

I have done all the steps as mentioned in txt file but don't know why calender don't popups when I select textbox. Kindly advice what could be reason

I am pulling my hair out with this exact problem. Maybe it doesn't play nice with ver. 1.5? I too followed the directions in the txt file and got nothing. Trying to see if I missed something obvious, and don't think that I did. Has anyone got any sort of calendar working for a product option??
