tpl_404_default.php Missing 1.5.0

I am getting this error

The ONLY way it would ever look for the NON-EXISTENT tpl_404_default.php file is if your site was pointed to http://your_site.com/index.php?main_page=404

And, since Zen Cart doesn't have a main_page=404 file, your post suggests to me that you're using a URL-rewriter that's malfunctioning and creating invalid redirects. Turn off your so-called SEO URL mod, and that problem will go away.
