UPS XML suddenly not returning calculated rates

We have been using the UPS XML mod successfully with the negotiated rates, but all of a sudden, all shipments are being charged $16 for shipping instead of returning negotiated rates. Has this happened to anyone else? Any ideas for fixing this?

Ok, just so everyone knows that I resolved this - if someone changes your UPS account password without telling you, the UPS module won't work anymore.. LOL
