Help with Delivery Option and fees

Not sure this is the right forum, but it's not an addon that I can find so this seemed the right place.

I have a florist as a client. They deliver to certain areas in KY, delivery fee is based on their zipcode. I feel certain this can be done, but I am at a loss. Below is what I'm looking at needing to do:
Please help me if you can. I'm having a major mental block!

40322 - Free
41472 - Free
40346 - $6
40387 - $6
40316 - $6
40380 - $12 Funeral Homes Only (is this possible?)
41301 - $12 Funeral Homes Only
41413 - $9

Thank you! I looked and looked and didn't see this one. I will give it a try now.

I have it installed. Have you used it? Do you know if the zipcode changes/zones need to be made in the database or if I did something wrong because you can't modify the zipcode zones in admin.

Does anyone know anything about how to use this addon? I don't know if I just put the zip codes directly in database or what to do.
