Trouble with flat rate shipping

I'm trying to setup the following:
Below $100, shipping is $5
$100 and up, shipping is free.

I've tried using the shipping table with the following options:
Shipping Table: 0:5.00
Table Method: price

And then using the Free Shipping Options module with the following option:
Total >= 100.00

But that doesn't work. All orders are shipped free.

I've looked on here and one other user had a similar issue but they resolved it on their own and didn't post the solution.

0:5.00 would mean 'Up to 0 lbs charge $5'
You should change it to 100.00:5.00

try that out.
