OK, did you install the plugin? What you attached is the plugin's zip-file, not the notifier_trace.log file that's generated by the plugin.

Indeed I did, then attached the wrong zip file. here is the correct one.

The contents of the file are kind of weird, implying that the page died in /includes/modules/pages/create_account/header_php.php but before the call to the /includes/modules/YOUR_TEMPLATE/create_account.php file. You could try a couple of things:
Try replacing the header_php.php file with the version of the file that came with the v1.5.1 distribution; perhaps it got corrupted in the FTP upload?
Replace the header_php.php file with:

where I added the line in red. If that line's present in the notifier_trace.log file next time, it indicates that you received control back from the require_languages.php file.
Delete the notifier_trace.log file before you do your next test, so that only the pertinent bits are captured. Also, please include which clicks you made to make it easier to 'follow' the trace in the file.

I will try this but do bear in mind that 6 new customers have created an account with this setup. It just 'broke' and i had not made any file changes only added a new product.

Software doesn't "just 'break'", something's changed and adding a new product won't affect the login/create_account process. Since the header_php.php file isn't getting control back from the include of require_languages.php, I'm guessing that something's awry in the language file for the create_account page.
If the file /includes/languages/english/YOUR_TEMPLATE/create_account.php exists, try re-FTPing it with whatever changes you've made. Otherwise, re-FTP the distribution version of /includes/languages/english/create_account.php and see if anything "gets better".

RESOLVED
/includes/languages/english/YOUR_TEMPLATE/create_account.php does not exist, it does not exist in the templates original files either.
I decided to just copy a stock 1.5.1 *create_account.php * to that location anyway. I have now created three test accounts in a row.
I would like to thank you very much for the help to resolve this issue, I was just about to reinstall the whole store, I was just at the end of my tether with it. So Thank you very much indeed. :smile:
