client says cart is auto matically adding attributes to their products

I have a client who is SWEARING up and down that their cart is auto matically adding attributes to their products. I do see products that have a color attribute and then ALL the colors that are options appear in the product page, although there should only be like 3.

Here is a summary of the site:

version 1.3.9 was done and I added a module called Stock by Attributes, but that did not do what I needed so I deleted it and then got a 3rd party mod, but that mod did not overwrite any core files other than invoice.php. and orders.php - everything worked fine for the client.

In October they decided they did not need that mod anymore so I removed it completely and removed the database fields and then upgraded the site to version 1.5.1 - I ran some tests and everything looked good.

A week or 2 later the client noticed that an item that had color options with 4 different colors assigned to it was showing EVERY color option that the store had in it. I said maybe it was a glitch from removing the attribute feature and putting the original zencart attributes feature back to use.....so had her redo the product options....

Today she emailed me again saying that this is still happening...it is random....one product I looked at had every color listed for the options.....this is an item that was added to the store in May of this year and should only have 4 or 5 color options...I checked the admin activity log to make sure no one touched that product - and no one did....

I just went through all the zen files for 1.5.1 to compare with the site and all looks good....

I know this is an out there issue, but I am stumped and clueless as to where else I can check into this?

Anyone have any ideas on other reasons this might happen.....I cannot wrap my head around it.....
