Problem;
Capcha is showing up on create account and works.
In Admin under Configuration there is a tab for CAPTCHA Validation. When I select it I get the configuration settings for the tab above it.(EZ-Pages Settings) I can open both above and below the captcha tab, but not the captcha.

I ran the sql from admin/tools and then with phpmyadmin after backup restore and full verification of files.

In my database at the admin_pages / structure it looks the same before and after the sql. There isn't any gID=31 added. Though both sql script runs reported no errors.

Did you pay attention to this in the install readme

--IMPORTANT!!! REGARDING the Installation SQL--

In the admin_pages table line, 4th section down in the SQL:

The page_params 'gID=31', and the sort_order '26' that you see in the 'admin_pages' table settings, are for EXAMPLE PURPOSES ONLY!!

If you are doing a fresh Install of Zen-Cart v1.5.0, with no other Add-Ons, added as of yet, you may leave these numbers as set in the SQL. However, if you have added additional Add-Ons already or, made changes to this table, you will need to set those numbers to what the next setting would be for them, in Sequential Order.

Found the error, easypopulate created an error in the db.
