Newsletter Subscribe Form

I can't seem to find a newsletter subscribe form or just a simply form with submit button to collect email address's for ver. 1.5+ There is one for previous versions but it specifically states not for 1.5+ Any suggestions or recommendations are appreciated, thanks!

Mailchimp is most likely your best bet, there's a sidebox mod in add-on area too.
