Having Two Flat-rate Shipping Options

I have been looking everywhere for this and can't find the answer.

How can I have two Flat-Rate shipping options, such as one for standard ground and one for priority overnight? Is this possible?

Also, is there a "call for quote" option if they're out of the US?
