Sale completing without required attribute

We are occasionally getting sales through where the crucial option in the form of an attribute is not being selected or somehow not carrying over to our order confirmation.

So for example the item will be a ticket to an event where you must select a date from a list which is setup as an attribute (and yes I have default value that is display only so you MUST make a selection in order to add to cart). Sometimes we get one through that is just a generic ticket, we have no way of knowing what actual event they wanted and have to contact them.

I cannot duplicate this error myself, I am baffled as to how this can happen.

Another example is we have Ebooks as downloads and sometimes we get a sale through that is just a 'download' with no Ebook title shown! In this case it's even more puzzling because each Ebook is a separate item listing, not a pull-down or radio button etc. The only common denominator is that an attribute is being used. :huh:

Sooooooo, any idea how such a thing can happen?

Weeeeeell, I guess no one has any idea how this can happen, it's a total mystery :-o

Have you URLs for some of these Products that this has happened on?

On the Ebooks, do they all show their Attribute(s) with a green dot on the filename?
