Password Forgotten page not working

When a customer clicks on password forgotten, they get a blank screen. I haven't changed anything on that page (or anything else password forgotten related), so I'm not sure what the problem is. I am using 1.5.1.

To make things easier as well, there is a plugin that allows viewing those logs from within the admin panel (not requiring ftp into the site). It is suggested that your site use a certificate in conjunction with the program as the data transferred may reveal information that is desired not to be captured as easily as without.
