Extra Insurance on Priority Mail not charged after USPS Update

Since the USPS update on May 31st, orders received are no longer being charged the extra insurance for priority mail shipments. For shipments valued over $50 (under $50 the insurance is free from the USPS), the module should automatically charge the extra insurance. For example, between $50 and $100, the added charge should be $2.75, in addition to the priority mail rate. In my shipping module, Priority Mail options are selected and under the Extra Services, the Yes box is checked.

My Zen Cart is 1.5.4, PHP 5.5. Prior to the update, all was working fine.

I contacted USPS and their reply was that all is working fine on their side.

In the USPS shipping module, down at the bottom, turn on the Debug to Log ...

Try your quote again and then Zip the shipping logs in the /logs directory and attach it to your post ...

You have Insurance checked ...

There are 3 checkboxes for insurance:
Insurance
Priority Mail Express Insurance
Priority Mail Insurance

If you want Insurance for Priority Mail check the box for:
Priority Mail Insurance

If you want Insurance for Priority Mail Express check the box for:
Priority Mail Express Insurance

If you want Insurance for everything else check the box for:
Insurance
