Define page disorder

Hi,

I created new "Defined Page" which runs when a payment transaction is unsuccessful. This page appears perfectly in Firefox and Edge, but not in Google Chrome. The page is moved to the left in Chrome for unknown reason for me.

Please can someone help me to figure out why this problem occurs in Chrome ? (please try to open this page in different web browsers)

When in chrome you open the console you will see three error about jquery not being able to load. This is probably the cause of the shifting.

What makes me confused is the next. Before I added this Defined page (checkout failure) I had created one exactly in the same way and that page appears in Chrome correctly. It's this page: http://infocom.co.me/demoshop/index.php?main_page=about_us

Let me use define_page_2 for this content instead of creating the new page. How I can rename that page in order to see the next link in address bar when page_2 is open: main_page=checkout_failure

I would like to see "checkout_failure" instead of "page_2" in address bar.

Fixed!

I found out the solution. I downloaded "About Us" addons from download section and renamed all file names to my page name and then uploaded it to my server. After that I got new page in Define page section.
