Packing Slip with images

I hve misplaced my Packing Slip with Thumbnail Image module, can anyone tell me wher eit is located or please send me a copy?

I found a copy on my PC - it works great

Just installed this module - works a treat - but it uses MODEL as the key to extract the picture, so if you do not use Model in your cart (as I was) - you will have to add them to use this baby.

Apart from that - pretty neat routine!!!

I have added the Invoice number and modified the attribute text to being large so it can be faxed to a drop ship supplier and elimiates the fax condesing it.
