Pricing Based on Attributes

I have a pricing scheme based on quantity. When the customer orders 25+ pieces, we offer imprinting (company logo, name, address). Pricing is different with imprinting than without imprinting. I would like to ask if the customer wants imprinting. If yes, I want Zen to prompt for logo, and address. I see the attributes available for the logo/address, but don't know how to ask if the customer wants imprinting. Also, I need to apply attributes ONLY to my product at quantity >= 25, and set price appropriately.

The attributes are applied to the product, not to the product when ordering quantities of 25+.

I apologize if this is explained somewhere that I've missed; I've searched the FAQs and looked @ WIKI. Any help, direction, and insight is GREATLY appreciated!!!
