Place to check off gift item/write gift message?

Hello,
I'm having some trouble putting in 2 options during the checkout process. (1) I'd like to put in a box where someone can check if they are purchasing the item as a gift and (2) I want to put in a box where someone can write a gift message.

If anyone has experience on how I can add this, I would appreciate it!

Any luck with this? I'd like to do the same thing.

This is an old request, but still relevant. Anyone know how to do this easily?

I need to ask for customer input for several things before an order can be processed, we deliver flowers. So we need to ask for the occasion, card text, what type of place it's going to (drop down), and special delivery instructions. Any ideas?

Just wondering if anyone knows of a mod in existence, or one that could be used for adding a gift/greeting card to products before checkout?

Nothing fancy, just something that could be checked and have text added to it.
I don't know why I thought there was one already, but I can't seem to find it, so it must have been either wishful thinking or I ate something too close to bedtime!

I had found that one prior to posting. I went to the authors help page and read about it but didn't see anything about a card being included. So I did think about posting in the thread asking if "card" could be substituted for "wrap", but then realized with my limited php knowledge there would be much more involved and chickened out.

I may still go back there and post, perhaps the mod author or someone else with some programming knowledge will have some "alteration" suggestions for me.
