Please help w/ shipping module

Hi. I'm obviously a newbie. I can't get the shipping module configured correctly. I want to do the following:

-Table-based shipping on orders up to $50.
-Free shipping on orders over $50.
-Only offer shipping to US and Canada.
-Free shipping applies to US orders only.

I defined a US-only zone to include all 50 states. Then I configured Zone Rates in shipping module. I also enabled Free Shipping Options. Everything else is disabled.

When I test it, I get a message that shipping is not available to that region. Does anyone have a step-by-step guide?

OK. I simplied it to just a flatrate shipping module, no zones, no tables, etc., and it still doesn't work!! Help!!

Free Shipping Options freeoptions is designed to display at the same time as the other shipping options based on the criteria you define ...

Free Shipping freeshipper is designed to display ONLY when the shipping is actually $0.00 as in Always Free Shipping ... or 0 weight when 0 weight means Free Shipping ...

NOTE: if 0 weight does not mean Free Shipping you need to change the setting in the Shipping/Packaging ...

Free Shipping on orders over $XX.XX is managed in the Modules ... Order Totals ... by editing the Shipping ot_shipping and setting the Free Shipping on for orders over XX.XX amount ...
