accepting donations and capturing shipping address

Hi all, I'm trying to figure out how to take donations with ZC and capture the shipping address so we have somewhere to send a thank you note to. I couldn't find anything in the forum so I figure I'm just not searching for the right thing. Any suggestions?

When they checkout / pay for the donation the cart will ask for email, name, address ect.
