Because my store sells a service, not a product, i need additional information before I let them complete their order. How can I add a page with check boxes for them to fill out?

Try using PRODUCT ATTRIBUTES - where the attributes are largely Text Boxes or check-boxes. When your "product" shows (in your case a "service"), these text boxes will show up underneath the description.
When you set up the text box attributes, make them "required" fields - forcing your clients to input some text. IF they bypass a required field, they can't progress to checkout - the shop will alert them to complete the field.
