multiple attributes - one download

I seem to be having a problem with mutiple attributes needing to be selected in order to download a single product.

On this page you have to enter the option to pick a team name and also the month.
How do I set up a download that will only be allowed when the specific options match?

If anybody knows if this is possible I could really use the help. If it's not possible... just tell me to stop asking and I'll move on to Plan B. It will take more time and I was hoping to avoid that but you can't win 'em all. Thanks

Currently there are not dependant attributes ...

Can you set the product up in a manner that makes it more logically correct so that there are not conflicting attributes?
