ENTRY_REQUIRED_SYMBOL on contact page

I have this showing up in my form labels on my Contact Page.

Is this an admin configure issue or template issue?

Sorry I should have said I have ENTRY_REQUIRED_SYMBOL showing in my form labels on this page: http://hoopbuilding.com/index.php?main_page=contact_us

I still could use some help figuring out where to fix it though :)

It appears you are missing or have several corrupted language files. You should compare all of the files in the /languages/ directory with a copy from the original download.
