Adding Radio/Checkbox to Delivery Page

On the Delivery Information page, I want to add a Checkbox, or Radio box that says something like "Autoship: Blah Blah Blah, please select" and depending on what they select, it will carry that YES or NO answer through to the confirmation email in the Office Use Only portion of the email.

So I just need to figure out how to get the code into both (or more?) templates so this will function properly. Thanks in advance!!

Is the Dr. in the house?

This isn't just a simple matter of adding a checkbox ...

Does the customer's choice need to be remembered with the order, so they can see it in their order-history, as well as allow you to see it in the admin ?

Or is it only needed in the email? If so, why are you running your order-processing only from emails ?

What is the meaning behind "autoship" in this case ?
If they check the box, what else is supposed to happen ?
