Adding extra fields to contact us page

I hope I am posting in the correct forum. I had asked this question last March and did not get a reply. I went to the downloads section and downloaded and installed the mod called Add_Phone_Number_on Contact_Us. The extra form field shows up nicely, but when I test the form, I do not receive the test email.

I've looked through other posts on this forum and tried the different instructions and explanations and I am still not getting the desired result. Some of these instructions are a couple of years old, so I don't know how relevant they are now with the current zen cart installation. I am quite frusterated.

What I actually need is to be able to add not only extra textfields, but also radio boxes and/or checkboxes as well and have the information pass into the email I receive as a test or to any admin email address that I enter into the admin email settings. I did make sure the correct address was entered into the admin email settings.

Will anyone out there please help?

Sounds like you're looking for more than just a "contact us" page.

The following link is to a discussion and ends with a packaged contribution that allows for multiple form inputs to be validated and emailed to the store owner. You could use it as an example to work from while designing your own custom forms:
