Is this type of mod available?

I searched the forum, but nothing came up. Forgive me if someone has asked this before.

I am looking for a shipping module that will do the following:

(1) On the Estimate Shipping pop-up, I want a shipping table. For example:

Purchase Amount ----- Shipping Rate:
$0-9.99 ........................... $2.00
$10-19.99 ........................ $4.00
etc etc.

And I want the module able to add the appropriate shipping charge based on the total purchase amount.

I would also like to turn off international orders, US shipping only.

That's what the built in table rate mod does.

To restrict shipping to the US, look at how zones work.
