Tell a friend form on product page.

Hi,

I have the "Tell a friend" button currently below the product description, but how can I display the form (email field and button) directly?

Thanks

I'm trying to install sidebox-anywhere addon to display tell-friend below the product description. I have the following code in tpl_product_info_display.php:

<?php
$zcSBmodule = 'tell_a_friend.php'; // name of sidebox or centerbox to be displayed (see filenames in the /includes/modules/sideboxes folder) (if not set, uses "whats_new.php")
$zcSBlayout = 'left'; // 'left' or 'right' sidebox template style (if not specified, uses 'left')
require ("single_sidebox.php");
?>

and getting this error:

Fatal error: Cannot redeclare class base in D:\xampp\htdocs\zen\includes\classes\class.base.php on line 17
