Checkout Help - Want No Login No Newsletter No Jive!

I'm trying to get the checkout to look cleaner -- my client hates the current checkout as our customers don't repeat buy and wouldn't want to log in (not as tech-savvy).

I have tried installing the My Checkout Without Account Mod, but that entirely erased all the shipping info on the checkout. So I now have gotten the cart somewhat back to normal, but as you can see its still broken.

Basically, what I want is...name, address, shipping, email and phone...then next page credit card...next page confirmation...then done.**

Also...is there a way to get an empty shopping cart simply to redirect to the last product viewed?
