Duplicate credit card fields on checkout page
Website Payments Pro is a credit card gateway, so will show you credit card fields the same way the Authorize.net module does. So, the "solution" to your "problem" is the same as described earlier: Click "Remove" on any modules you're not actually using.
ie: if you've not subscribed to Pro, remove it.
And, if you're using Express, then you should also click "remove" on Standard.
